A rare, highly unique, complex emergency valve replacement surgery was performed by the doctors at the KMCH and post surgery, the patient is also doing well, informed the team of surgeons
Coimbatore: The surgeons at the KMCH, Kovai Medical Centre, and Hospital performed a new, but complex surgery, that was not an open heart on a 65-year-old patient.
Calling it a rare, unique Emergency Valve Keyhole Replacement, the doctors state, “These procedures require detailed planning and so emergency procedures are seldom performed.”
Transcatheter Valve replacement is a relatively new procedure for replacing diseased heart valves without open-heart surgery. Replacing a previously implanted surgical valve is more complex and very high risk.
A 65-year-old man was transferred to Kovai Medical Center and Hospital from a peripheral hospital with sudden onset chest pain and breathlessness after lifting heavy weight while working in his field. He had previously undergone a valve replacement surgery 11- years ago.
At KMCH, he was immediately connected to a ventilator as he experienced severe breathing difficulty. Emergency Echocardiography and cardiac CT showed avulsion of the valve of the previously replaced valve with the severe leak of the blood. Chest trauma or sudden heavy straining can sometimes cause this, and most patients do not even get time to reach a medical facility.
Since he was very sick and an emergency re-do open heart surgery was deemed to carry a very high risk, he was offered emergency Transcatheter aortic valve implantation - TAVI (valve in valve procedure). In this procedure, a new valve is deployed inside the previous valve by endovascular method from a keyhole opening in his thigh without the need to open his chest or use a heart-lung machine.
The KMCH structural heart team comprising of Dr. Thomas Alexander, Dr. Balakumaran, and Dr. Prashanth Vaijyanath swung into action and performed the procedure successfully, within 2 hours of patient presentation since delaying the procedure will be harmful to the patient. He was extubated from the ventilator the next day and discharged on the fourth day.
In a statement, the city-based hospital informed that, this is a rare instance of an emergency TAVI done for a central valve leak that happened suddenly due to rupture of valve leaflets within a previously surgically replaced valve.
